Why don't we get post-it pads like this at work? I would envision the meeting in which they hand these out sounding something like this,
"In order to better understand the team you will be working with for Project X, please fill in your own blanks and checkmarks on the attached report. If it's been that kind of morning for you go ahead and draw yourself under the appropriate section (coloring optional, markers located on the second shelf of the supply closet). Once your report is complete we'll reconvene to discuss each others work, our expectations as a team and how best we can utilize each individuals talent."
